
Neighborhood House Association's Case Management and Peer Support Services program helps clients maintain and improve health wherever possible and gain access to resources and services that meet each client's individual level of need. The program provides comprehensive, ongoing assistance to individuals with HIV/AIDS. Case Management staff advocates on behalf of clients to ensure proper treatment and care. The program consists of three components: intensive case management for ex-offenders, general case management and peer advocacy.
Transportation Services
Neighborhood House Association's Transportation Services provides door-to-door assisted and unassisted transportation to low-income residents of San Diego
County who are affected by HIV/AIDS. Unassisted services refer to bus passes and coaster passes that are provided to clients via case management programs.
