Neighborhood House Association's Adult Day Health Care Center (ADHC) is a day treatment program that provides nursing, rehabilitative therapies, nutrition, psycho-social services and recreational activities to adults 18 years of age or older who are experiencing chronic physical and/or mental health conditions.
Payment
ADHC is a fee-for-service program. We accept, but are not limited to, private insurance, MediCal and Veteran benefits.
Eligibility
ADHC is open to adults, age 18 and older, whose needs include:
- On-going basic nursing care and/or supervision (such as diabetic care management)
- A medical condition that requires treatment (such as rehabilitative therapies)
ordered by a physician
- A memory impairment such as Alzheimer's Disease or dementia
- Chronic health problems that affect the ability to perform daily living activities
